Variation of a designated premises supervisor

Only the holder of the Premises Licence can make an application to vary the Designated Premises Supervisor. If the Designated Premises Supervisor leaves the licensed premises then a variation must be made immediately. You will not be authorised to sell alcohol until the correct application has been made. You will need to obtain consent from the new Designated Premises Supervisor.

The Designated Premises Supervisor must complete the  Consent of individual to being specified as premises supervisor (PDF, 43 KB) form. This will need to be uploaded to the online form so you will need to have had this completed before you complete the application to vary a Designated Premises Supervisor.

You will need to provide the current premises licence or reason why it cannot be provided.
